How to Appoint a Representative

Navigating Medicare can be difficult. If you are an IU Health Plans member without a legal representative, or Power of Attorney, you can give permission for another person to act as your representative. There are two types of representation:

Both you and your representative must sign the forms for them to be valid. Please see additional instructions within the forms.

If additional help is needed, please call the IU Health Plans Customer Solutions Center at call 800.455.9776 (TTY/TDD 711). The hours for the Customer Solutions Center are: 8 am to 8 pm, Monday - Sunday between October 1 - March 31 and 8 am to 8pm, Monday - Friday between April 1 - September 30.
